I wanna like this but the dialog, or exposition I suppose, in the first half seems pretty unnatural to me. And even that aside…

SCP-3436 doesn't currently exist as a page on the SCP wiki, which makes me think about what happens if someone makes an SCP with this number. Does the author of this Consent form just change the number of the form or does he plan on writing SCP-3436 soon?

This is good. Storytelling via in-universe documents juxtaposed with prose is a favorite for me.
