It's… okay? I don't see any reasons of why this should break the format tbf. Until I get an explanation that satisfies me, it's a downvote for me.

I don't know if this will satisfy you or not. Still, the format break is that the person writing the article is 6892-1. It is to imply that the SCP is already established, and the anomaly and the idea of having the writer of the report in-universe being an active agent in the story being told.
And the way I had this setup, if I used the standard format, it wouldn't work.
Oh, don't get me wrong, I do get it, still I really don't see the Foundation make a Class E (I guess before actually being an SCP the ghost would be classified as Class E) write their own document. I also don't feel particularly touching the prayers and attendance part, because the researcher's still technically there, they could… just meet them, I guess?
I like this a lot. It's sweet and doesn't overstay it's welcome, definite +1 from me.

While I do agree with Siddhartha, I'm very susceptible to the pathos of maintaining respect for the dead.

I know I'm nitpicking, but I feel that this line was a little too tell-y. Like an announcement of, 'I've died and these two people are good to me.'
A slight bit more show, a slight bit less tell, and this piece would be stronger.
While I personally don't mind the format screw, I also think having it be a full document with the ghostly thank you from -1 in the Addendum would have been better.

I see Harmony lent you a hand in critting this article. I'll leave out any personal comments here; we all know her history, and we all have our own opinions.
However, I must say this:
Author, I respect the fact that you stick to your principle and give credit where credit is due, even if a name isn't too fondly remembered. I respect the shit out of that. That takes guts.
