I am unable to believe that a physician would not only write things that are "outside of hospital code" and sign them, leaving open the possibility of those words being used against them, but also that they would do so on facility letterhead.
This is intentional, it's absurd a genuine doctor would write this. I go towards the explanation of the man slowly and unknowingly affecting this doctor as his abilities started to manifest, making him out to be the caricature that he is in his mind.
The concept is about a man with an extreme denial of reality that it becomes anomalous, and starts affecting the world around him. The opposite of a psychiatrist wanting to help and encourage recovery would be a resentful fed up doctor that believes what the patient thinks of himself. In this case, "I'm a hopeless case."
The letter is, in a way, the character talking about himself. If that makes sense lol.
