My notion is there's some abandoned missile silo that became a privatized research lab where some sort of dimensional portal was opened or some other extremely localized but mysterious threat came into being, possibly of a mystical nature instead of just pseudoscientific. When SCP is contacted by the local government to investigate the situation, there's one mutated (or something) survivor or something in there, and that person claims the situation seems currently under control but that opening the containment area will be extremely dangerous as would moving that survivor somewhere else. Analysis by the research team confirms the survivor's report and that whatever containment in place needs to be reinforced indefinitely, which is the reason why the Foundation doesn't just leave, investigate directly, or neutralize the site. Because it basically just means having guards on hand and providing power to the laboratory, the personnel is switched out largely with less expensive trainees and the guards become extremely bored and start messing about. The person in the lab becomes progressively more pissed at the bunglers up top, which possibly somehow results in a temporary containment breach. There'd be the usual matter-of-fact "Containment" and "Description," with all the story in diary entries.
